Although Kaleidoscope’s programs are free, you still need to register.

You can register your child(ren) by clicking on the Registration Forms below and printing them out, or by calling (612) 871-9268 and requesting a registration form.

Children may register for one or all of the programs, however, space is limited. Registration is on a first come, first served basis.

2007-2008 School Year Registration

The 2007-2008 Kids’ Place and Youth Village programs run from September 10th, 2007 through May 23rd, 2008. Registration is currently open.

2008 Summer Kids Registration is Now Open!

Summer Kids 2008 begins on Monday, June 16th and ends on Thursday, August 7th. The program runs Monday-Thursday from 8:30am-4:00pm. We are closed on Fridays.

Registration for our 2008 Summer Kids program is now open. Updated forms for Summer Kids registration have been posted on the website. You may also stop by the office to pick up the forms. Kaleidoscope Place Registration

Thank you for choosing Kaleidoscope Place’s programs for your child(ren). The New registration forms allow you to register for our Kids’ Place, Youth Village, and Summer Kids programs. We have made some changes to our registration process. Please read the following information carefully. Once you have completed all required documents, you may mail, fax, or drop them off at our office. Our fax number is (612) 871-0173. Your registration is not considered complete until we have received all completed, required documents. Once your child(ren) has been accepted into our program, we will notify you with a Welcome Letter along with a program calendar and our Behavior Expectations Guidelines.

  1. Please complete all sections of pages 1-7.

  2. Student ID numbers must be completed on page 1 of the registration packet. If you do not know your child(ren)’s student ID number, please contact their school to obtain it.

  3. Included is a School-day Teacher Assessment. Please give this to your child(ren)’s classroom teacher and have them complete all sections. The teacher must complete this page and it must be returned to Kaleidoscope in order to complete your registration.

  4. Included is a Summer Food Service Program-Household Income Statement. If you are registering for our summer program, please complete both sides of this form to help us provide nutritious meals for your child(ren).

  5. For returning families, you must complete the Kaleidoscope Place Returning Parent-Caregiver Survey 2007-2008 and return it with pages 1-7 of your registration packet. Click on the Kaleidoscope Place Returning Parent-Caregiver Survey 2007-2008 link below to print it out.
